Requirements:
1. Must have a prescription from your doctor stating the following:
Diagnosis
Duration of treatment (i.e., 2 times per week for 6 weeks)
Treatment (must specifically state Massage Therapy as the treatment)
2. Complete the Insurance Information Form (go to the "Helpful Forms" tab)
3. Must sign a Privacy Notice
Instructions:
1. Client obtains a prescription from their doctor.
2. Client fills out an Insurance Intake Form and makes a "tentative" appointment for two weeks away.
3. Soothing Ways Massage Therapy will contact your insurance carrier to confirm coverage for massage therapy, what percenage of the bill they will pay, and what the co-pay amount is that the client will pay. Once confirmation is received the scheduled appointment may take place.
4. The client is required to pay any co-pay amount due at the time of each session.
5. IMPORTANT: the client is responsible for any unpaid amount the insurance company does not pay. This means, even after the client has paid their co-pay amount, if the insurance company does not pay any or part of the remaining amount due, it is the client's responsibility to pay all unpaid amounts.
6. If you can't wait two weeks to get confirmation and need a massage immediately your session(s) must be paid in full at the time of your session. After insurance confirmation has been received you may submit a claim to get reimbursement.
